2134 Issuance of a certificate for insurance income (UP 2)

Requirements, procedures, instructions

Form and manner of ordering the service

A written application shall be submitted in a form in one of the following ways:

     - personally - with an identity document;

    - by other persons - with an explicit written power of attorney with notarized signature, or by officials under Art. 40, para. 3, Art. 54g, para. 1, art. 98, para. 1 of CSR;

     - licensed postal operator / post office / - the application should be in original;

     - by courier service - the application should be in original;

     - electronically by email: Виж имейла...;

     - through the Secure Electronic Service System (SESS).

When applying, the person must indicate the desired way of obtaining the certificate.

Necessary documents

       - Copies of employment / service book;

       - Document for identity of names, in case of changes;

       - Notarized power of attorney, when the request is not submitted personally by the interested person - in original or copy.

The original accepting documents shall be submitted to the official accepting the application for verification.

When the service is requested electronically (at email Виж имейла...), the application, the copy of the employment / service book concerning the period of certification УП-2 and other accompanying documents (if applicable) should be signed with a qualified electronic signature (QES).

Procedure for providing the service

The applicant submits an application form to the Executive Director of AGCC for issuance of a certificate (UP-2). When applying, the position he / she has held and the period for the insurance income shall be indicated. The application, together with the attached documents, is entered in the record keeping system and is resolved for the Administrative, Legal and Financial Services Directorate, Financial and Economic Activities Department (FSD) for the issuance of the respective document. An employee of the FSD department performs an inspection and issues the document according to Art. 40, para. 2 of the NPE on the basis of payrolls, other supporting documents and contracts for assignment of labor, stored in the archives of AGCC. The document is signed by the person who prepared it, the chief accountant and the executive director.

Method of obtaining the result of the service

The certificate is received:

  - in person - against an identity document;

  - through a licensed postal operator / post office / with a return receipt;

  - through courier service;

 - by other persons - against an explicit written power of attorney with notarized signature / original or copy, the original being presented for verification /.

The costs of sending through a licensed postal operator / post office / with a return receipt and courier service are at the expense of the applicant.
